How do you apply?
It's easy to complete an online application. First, collect the following materials:
A copy of your driver's license with your current address (if not current, provide proof of address).
Beneficiary information: each HSA account will have a beneficiary, to complete the application please provide his or her name, date of birth, address, Social Security number and phone number
To allow someone else access to your HSA benefits you can add them as an Authorized Signer. You will be asked to provide that person’s name, Social Security number, date of birth, occupation, employer, mother’s maiden name, driver’s license number with state and expiration date, and phone number or email. The Authorized Signer will also sign a separate form.
- The application will ask for an initial $5 deposit. To bypass, select Internal Transfer and enter 1111 for the account number.
You will receive an email from Cinfed confirming your application as well as a link to sign documents electronically and finalize your account. Cinfed will coordinate your account with CAC’s HR Department, and you will receive your HSA Mastercard debit card within 14 days.
